The Health Care for Homeless Veteran (HCHV) social worker is assigned to a HCHV team within Community Reintegration Service (CRS) at the Community Resource and Referral Center (CRRC) under the Mental Health Division. The CRRC HCHV social Worker will work at CRRC walk In Clinic that is co-located with Homeless Primary Care Team in downtown Portland and is responsible for providing assessments, resource and referral services, case management and mental health services to homeless Veterans.

Completes biopsychosocial assessments to assess functioning, psychosocial status, and basic needs of patients and their family members to formulate and implement a Veteran driven treatment plan.
Provides assessment and treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversifies backgrounds.
Uses knowledge of human development and behavior (physical and psychological) and differential influences of environment, society and culture when completing assessments and developing intervention strategies with Veteran, Caregivers, and multidisciplinary team.
Maintains and grows knowledge of psychosocial treatment and implements evidence-based treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical and social problems to assist Veterans and Caregivers achieve treatment goals.
Provides consultation services to other staff about the needs of patients and the impact of psychosocial problems on health care and adherence with treatment.
Completes suicide risk assessments and complete all required clinical care and documentation associated with identified risk.
Work Schedule: Monday - Friday, 0800- 1630
