The LPN will support the various clinics within Specialty Procedures Care Clinic: Urology, Wound Care, Dermatology, Foot Care and Same Day Access. The LPN will be expected to be proficient in the application of complex wound dressings, gather specimens for pathology, set-up procedure rooms for minor invasive procedures, insert straight catheters and Foley catheters, and set-up procedure rooms for Urology procedures (i.e., cystoscopies and prostate biopsies).

Clinical communications and care with patients will be documented in the electronic medical record. In addition to direct patient care, the incumbent may have the opportunity to further their professional development by managing collateral programs. Initiates, performs, and completes assigned duties including implementing orders and providing care to variable patient populations. Recognizes the unique needs of different patients and how these vary according to age and/or illness. Performs support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during and following endoscopic procedures. Demonstrates knowledge to identify and responds to the patient's needs for care, including medication, patient safety, equipment assisted care and patient/family education; documents per policy. Functions as a technical expert in the use of the Specialty Care Clinical Procedure equipment and the procedures for set up, cleaning, reprocessing and storage of cystoscopes
1. Functions as an assistant for complex Specialty Care Unit procedures.
2. Functions as a primary staff responsible for the handling of the instruments, supplies and equipment during procedures.
3. Prepares procedural rooms for urology, dermatology and wound procedures.
4. Sets up sterile fields with all required items for each procedure
5. Troubleshoots common basic equipment problems and appropriately labels and delivers equipment to Biomedical Engineering
6. Performs non-technical maintenance of equipment to prevent malfunctions.
7. Identifies equipment needing preventative maintenance or repair and takes appropriate action.
8. Assesses equipment needs and maintains adequate stock and supplies and materials.
Responsible for assisting the SCC team with ongoing, continuous care of Veterans.
1. Functioning at the full extent of the team member's scope of practice based on experience/training.
2. Participates in team performance improvement and sustainment activities to enhance care delivery.
3. Participates in process improvements that enhance the care for the Veteran; access, Veteran centered care.
4. Manage communications and facilitate safe transitions of the Veteran between the SCC and facilitate
health care setting using warm hand-off communication either in person or by telephone.
5. Provide health education on wellness, disease prevention, chronic care management and self-management skills to patients and caregiver,
6. Encouraging patients to engage personal support persons in care and receive input from patients and personal support persons
7. Use informal/formal communications that are respectful, effective, and timely with all team members and other internal/external customers to convey significant, information for the care of the patient.
8. Also responsible for specific role assignments, including: Assists with pre-visit phone calls to Veterans to complete reminders as appropriate and obtain reason for appointment if not indicated. Reviews daily clinic schedule for patient care needs. Uses motivational interviewing and applies TEACH techniques to guide patients in setting goals.
9. Manages clinic flow which includes the following: a. Vital signs and treatments as indication/appropriate. b. Assisting the provider with exam/procedures. c. Administering and tracking immunizations (per licensure). d. Preparing and administering medications (per licensure and protocol). e. Completes orders per provider or protocol. f. Keep providers on schedule. g. Keep Veterans advised of delays/wait times. h. Observes, records and reports patient's response or changes in patient's conditions. i. Promptly recognizes and responds to urgent or emergent situations. j. Assists with orientation of new employees. k. Checks daily equipment/supplies and restocks the exam rooms. l. Acts as patient advocate; assuring patient understand content of visit and when to return to clinic, as well as how to contact team between visits. m. Promotes patient and personal safety.
10. Assists Specialty Care RN in coordinating care for patients when Veterans are: a. Receiving care from PACT Providers. b. Walk into the clinic without an appointment (gather data and patient care). c. Receiving care from community care providers (dual care). d. Needing relevant information communicated to involved providers. Prepares and administers prescribed medications (oral, subcutaneous, intramuscular) and performs treatments according to established policies/procedures. Reports medication errors, reactions, and side effects immediately appropriately and by National Patient Safety regulations. Prepares and processes Reusable Medical Equipment (RME)
