Disability Support Specialist |

The Disability Support Specialist plans, implements, and monitors response to information access needs of programs and services administered by the Community Supports and Access Section of the Disability Services Division. In addition, this position will provide a coordinated response from external customer and stakeholder inquiries generated from the Disability HUB MN and Disability Response Systems.
Job Responsibilities include:
•Manage and respond to information and assistance requests from the Disability HUB MN system and Disability Services’ Disability Response Team.
•Conducts related training, communication, outreach, and systems management activities to meet project/program goals and conducts other duties as required.
•Coordinate the implementation and maintenance of the Disability Hub MN training plan.
•Participate in integration team to develop, implement and monitor protocols, for providing Disability Hub and Disability Response Team services, to DHS, other state agencies, and providers, to support the projects’ and the Divisions’ strategic goals.
Qualifications & Requirements
Must meet one (1) of the following education or experience qualifications listed below:
•Two (2) years professional experience managing, and training and responding to information or assistance requests for a disability services delivery program; OR
•Bachelors or higher degree in public administration, vocational rehabilitation, psychology, social work, human services and one (1) year of professional experience managing, training and responding to information and assistance requests for a disability services delivery program.
Experience must demonstrate.
•Customer service or call center work responding to information and assistance requests;
•Knowledge of disability waiver programs and services or Minnesota/or other State Health Care Programs; and
•Coordination, the implementation, and maintenance of a training plan.
•Experience working with health care systems, counties, providers and advocacy organizations within the disability community.
•Experience working with people with disabilities.
•Experience working with MMIS and MAXIS systems.
•Knowledge of federal benefit programs, including Social Security, Medicare, and work incentives.
•Experience using Microsoft Dynamics Customer Relations Software and SharePoint.
