<p><span style="text-decoration: underline;"><strong>ABOUT THE PROGRAM: </strong></span><strong> Young Adult Intensive Community Services (YA-ICS)</strong></p>
<p>YA-ICS is a Multidisciplinary team comprised of the Program Director, Community Support Specialists, Employment Specialists, RN, Outreach Therapists and Care Coordinator. The mission of YA-ICS is to assist young adults in obtaining the skills they need to lead independent and productive lives. The team provides skill teaching in the areas of life skills, coping skills, symptom management, interpersonal relationships, vocational and educational goals and health and wellness. The team utilizes assertive engagement interventions in order to address barriers to treatment and meet the young adult “where they are at”; outreach is the main mode of service delivery. YA-ICS emphasizes teamwork and meets a minimum of three times a week as a full team to coordinate care and to support one another; staff also receive individual supervision with the Program Director who is a Licensed Mental Health professional with an expertise in working with the young adult population. Many of the YA-ICS clients have experienced significant and/or complex trauma and the team utilizes trauma informed care.</p>
<p><strong><span style="text-decoration: underline;">ABOUT THE POSITION:</span> Community Support Specialist II</strong></p>
<p><strong>Duties & Responsibilities </strong></p>
<ul>
<li>Partnering with the individual to accomplish their recovery goals.</li>
<li>Conducts Co-occurring Screens, C-SSRS and Functional Assessments.</li>
<li>Collaborates with individuals to create individualized, person-centered treatment plans and documents clients progress during treatment.</li>
<li>Use Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery, utilizing stage wise interventions.</li>
<li>Provide psycho-educational rehabilitation services via group and individual modalities.</li>
<li>Support individual with linking and engaging with natural (family, friends) or community supports.</li>
<li>Develop crisis plans and Safety Plans for clients and work with other staff to respond to crisis situations as needed.</li>
<li>Provide skills teaching, such as assisting clients in obtaining/ maintaining housing and utilities; learning how to use transportation services, planning and preparing nutritional meals, engaging and communicating with family and friends, safety in the community, caring for children and other family members, and solve everyday problems that arise. Participate in family education and support activities.</li>
<li>Documents client progress per agency policies.</li>
<li>Attend and participate in staff meetings, trainings, case conference, supervision and other meetings as assigned.</li>
<li>May perform other assignments and duties, as assigned.</li>

<p><span style="text-decoration: underline;"><strong>QUALIFICATIONS:</strong></span></p>
<p><strong>Education:</strong> Bachelor’s degree or equivalent education and/or work experience required.</p>
<p><strong>Experience:</strong> 1+ years working with identified population preferred.</p>
<p><strong>Licensure/Certification/Registration:</strong> Valid driver’s license required</p>
